The incident was an unauthorized intrusion on or around May 18, 2025. It was discovered May 26, 2025, with patient notices starting around July 11, 2025. Covenant Health later reported that more than 478,000 individuals were affected.

What do payments cover?

Cash Payment A pays up to $5,000 for documented, unreimbursed losses caused by the breach. Useful proof includes bank statements, receipts for credit freezes, postage, mileage, and time records. Only losses tied to the breach qualify, and documentation is required.

Cash Payment B is an estimated $75 payment requiring no proof of loss. According to OpenClassActions, claimants may seek both documented-loss and no-proof payments under the settlement terms. According to ClaimBee, cash amounts may rise or fall pro rata based on claim volume and court-approved deductions.

How do you file on time?

According to ClaimBee, valid claims must be submitted through the cash claim filing deadline process online by 11:59 p.m. Eastern on Dec. 30, 2026, or mailed postmarked by Dec. 30, 2026. Online filing uses the Class Member ID from the postcard through administrator Kroll.

Keep a copy of the confirmation or mailed form. Justice Bruce C. Mallonee granted preliminary approval Sept. 15, 2026. No final-approval or payment date is set, so payments will follow later court action.

  • Find the postcard with your Class Member ID
  • Choose Payment A, Payment B, or both
  • Attach proof for any documented-loss claim
  • File online or mail before the Dec. 30 deadline

Do you get monitoring or other options?

Class members also receive two years of CyEx Medical Shield Complete credit monitoring through an enrollment code. According to eosguide, enrollment opens after final approval rather than at filing. Save the settlement notice until codes become available.

Opt-outs and objections are due Dec. 15, 2026. Opting out preserves a separate lawsuit but gives up settlement benefits. Objecting keeps you in the class while asking the court to reject or change the terms.
