What Is the Case About?
An Arizona cardiology practice agreed to settle a data breach affecting roughly 484,000 individuals. The breach, discovered September 29, 2023, exposed names, Social Security numbers, dates of birth, driver’s license/state ID numbers, health insurance details, and medical information after a threat actor gained access and deployed ransomware.
Who Qualifies?
U.S. residents whose information was compromised in the September 29, 2023 breach and who received a notice with a Class Member ID by email or postcard.
How Much Can You Get?
- Approximately $75 pro rata cash payment (no documentation)
- Up to $5,000 for documented out-of-pocket losses traceable to the breach
- Or two years of medical monitoring
Combined benefits cannot exceed $5,000 per claimant.
