You need five packets: a signed IRS Form W-9, system IDs and contacts, a list of each well and plant, lab PFAS results with numeric values, and flow-capacity records. Forever chemicals means PFAS, the contaminants behind national drinking-water settlements for U.S.
public water systems. The official AFFF MDL 2873 settlement site routes water-system claims through its Claims Administrator claim filing portal. You must select Phase One, Phase Two Action Fund, Supplemental, or Special Needs.

Who Can Use This Form?
These forms pay U.S. public water systems with detectable PFAS. The Environmental Finance Center Network reports about $10.3 billion from 3M, $1.185 billion from DuPont/Chemours/Corteva, plus $750 million committed by Tyco settlement overview. Private wells and individual households do not qualify here.
If you are a resident, do not file this form for illness. Lawsuit Legit reports no global personal-injury settlement existed in MDL 2873 as of Aug. 28, 2026. Residents pursue separate deals, such as Hoosick Falls, with medical, residency, deed, well-water, and blood-test records.
- U.S. public water system with a federal PWSID
- Detectable PFAS in a well or treatment plant
- Filing for testing or treatment costs, not personal illness
What Identity Papers Are Required?
Every claim needs a duly completed and executed IRS Form W-9 for the system as to each settling defendant. The Official DuPont Public Water System Special Needs Claims Form requires the same W-9 plus a Treasury-regulation statement for Special Needs claims. Use the exact legal name and address on file with the IRS.
List system identity and contacts, federal PWS identification number, and employer identification number. The Official Tyco Special Needs Claims Form calls for those identifiers on every filing. A missing PWSID or EIN slows review.
What Lab Proof Must You Attach?
Provide detailed laboratory results with actual numeric values for each source. The Tyco Claims Form Addendum and Instructions call for untreated raw water results in most cases, including PFOA, PFOS, and other qualifying analytes Tyco testing instructions. Estimates or summaries without values do not count.
Test each groundwater well and each surface-water treatment plant you own or operate. Use a separate entry for each source. Keep chain-of-custody sheets and lab names with the result pages.
How Do You Show Flow and Capacity?
Report flow capacity for each impacted source in gallons per year, gallons per minute, or million gallons per day. The Tyco Allocation Procedures require verification against maximum flow and the three highest annual averages from 2014-2023. Pull numbers from operating logs, not design ratings.
Match each flow figure to its listed well or plant. A plant with two intakes needs two matched sets. Mismatched sources and flows trigger corrections.
When and Where Do You File?
File electronically or by mail through the court-approved Claims Administrator, then track the phase track you selected. Keep copies of the form, W-9, IDs, labs, and flow sheets together. Special Needs filings add the Treasury statement.
Phase Two systems must baseline-test every well and treatment plant. Texas Municipal League reports a Jan. 1, 2026 testing-cost claim deadline, then DuPont action-fund claims by June 30, 2026 and 3M claims by July 31, 2026. Missing a track deadline limits that track payment.
