Granite Wellness Centers) following a January 2021 ransomware attack that compromised the personal and medical information of up to 15,600 individuals. Affected individuals may file a claim by April 27, 2026.
What Happened?
Granite Wellness Centers experienced a ransomware attack in January 2021 that exposed sensitive patient information, including names, dates of birth, addresses, Social Security numbers, health information, treatment details, insurance information, driver’s license numbers, and bank account numbers. As a substance use disorder treatment provider, the compromised data includes particularly sensitive medical information.
Who Is Eligible?
You may be eligible if your personal information was compromised in the Granite Wellness Centers data breach. Affected individuals should have received a breach notification letter. Up to 15,600 individuals are covered.
What Benefits Are Available?
Eligible class members may receive:
- An estimated ~$750 pro-rata cash payment
- Up to $5,000 for documented out-of-pocket losses
- An additional $100 statutory payment for California residents at the time of the breach
